You can set up questionnaire (questions) electronically for
evaluating courses, college procedures, teacher�s class performance etc to
obtain feedbacks from students to improve courses to better streamline the
college processes. Questionnaire/survey only appears in specific user portal as
per their user assigned.
10.4.1.1 Hover
your mouse over Communications main menu and click Survey
Management as shown in figure 10.4.1a. You will go to the page as shown in
figure 10.4.1b.

Note: You
also have option to choose the form type from the drop down menu in figure 2 .The
College may use the form type Evaluation for evaluating its business
process, class performance, teacher�s performance etc� whereas AQTF Survey
form type is a government requirement which college must submit to the
government at some specified date/period.
10.4.1.2
Select the Form Type. Example: AQTF Form as shown in figure
10.4.1b.
10.4.1.3
Select the Form name from the drop down menu for e.g. AQTF
Form-Learner Questionnaire/Student Survey. If the form name you are looking
for is not available in the drop down menu you can create your own form name by
clickinglink as
shown in figure 10.4.1b.

Select the course type for which you are going to do evaluation.
Example: ALL as shown in figure above.
10.4.1.5
Select the question heading from the drop down menu. Example: ABOUT
YOU as shown in figure above. You can also create a new question heading by
simply selecting �Add New� from the drop down menu
10.4.1.6
Select Question Type from the drop down menu. Example: Check Box
as shown in figure 10.4.1b. Question Types determine the format of the
questionnaire that is visible in the student portal i.e. whether the questions
should appear in the text format, check box format or in the radio button
format. You can also create a new question type by simply selecting �Add
New� from the drop down menu.
10.4.1.7 Select/enter
value for question type in step 10.4.1.6. Example: Excellent, Good,
Satisfactory, Poor as shown in figure 10.4.1b. The value for question type
is basically the answers the students will choose for the corresponding
question. If the values are not available in the drop down menu you can create
your own values by clicking link as
shown in figure 10.4.1b.
In step
10.4.1.7, after clicking, you will
see a pop up window like the one below
Number of
options: Depending on how many options you are going to add, e.g. as seen
above we have four different options text so you would select 4, however if you
only have three option text then simply select 3.
Enter
options text seperated by comma: This is where you would enter the text
answers that the students can choose from, Please
note: that you must enter values followed by a comma.
Enter
options value seperated by comma: You can simply have the same as the field
above or you can specify numbers or letters e.g. 1, 2, 3, 4 or a, b, c, d. Then
simply click on OK to add.

Finally enter question for the questionnaire as shown in figure 10.4.1b. These
questions will appear in the questionnaire in the student portal for students
to answer them.

Finally click in
figure 10.4.1b.� Your recently added questionnaire will appear in the list as
shown in red box in figure 10.4.1b.

10.4.1.10 If
you want to have a preview of how your questionnaire would appear simply
click the �icon as
indicated by the red arrow as shown in figure 10.4.1b. Your preview would
appear as shown in figure 10.4.1c.
10.4.1.11 You
can also change the Question Order, which allows you to organize your
questions by having the important ones first, simply select the number as
indicated by the blue arrow as shown above in figure 10.4.1b then click on the icon to
update.
You can also
edit/delete the questions by simply clicking on the icons as
seen above in figure 10.4.1b

Similarly you can add as many questions to your
questionnaire by following the same steps as discussed above.
�This icon
which is shown above in figure 10.4.1b will allow you to complete the survey
and submit it.

which is shown in figure 10.4.1b allows you to send a request to students so
they can complete the survey. Please refer to section 10.4.5 Invite Student
to Submit Survey.
